This form is for logging all opening, closing, and periodic maintenance procedures for the functional testing room.

Weekly Maintenance Checklist

Performed weekly self-tests.

Belt inspected for alignment and signs of wear.

Please note any issues, damages, or items left for the opening team, and remember to submit these as an incident.

[ ] Auto BP Monitor: Tubing inspected for cracks or leaks.

[ ] Polar H10: Straps inspected for wear and tear at connection points.

AED has performed monthly self-test.

[ ] Replace batteries and test grip strengths/handle adjustments (link below for step by step)

[ ] Sensors: Check and calibrate sensors to ensure correct weight ( link below on step by step process)
